Coming out of hospital it doesn’t always mean someone is ready to go straight back to the life they lived before. Reablement is for when people need a bit of temporary extra support at home.
The result is someone less likely to be re-admitted to hospital or needing long-term care. This service is variously called ‘short term intervention care’, ‘intermediate care,’ or ‘Reablement’. It can be at home, or in one of Care Dorset’s sites.
It’s about ‘doing with’ rather than ‘doing for’ – working together on a path back to independence, regaining key skills, confidence, and freedom.
People are supported with cooking, health & hygiene, moving around their home safely, and in managing daily activities.
